
Gynecomastia is considered male enlargement of grandular tissue often followed with excessive fat tissue in and around the gland.
Such change can cause increased sensibility, enchanced areola and different levels of excessive skin.
Most of men population in some period of puberty experience the enlarged breast tissue that passes as they grow, while in some men the matter remains and intensifies with age.
If change occures early in the teenage period, there is no need to hurry with the surgery, but only follow the condition in case of any possible changes. If change occure suddenly in elder men, it is important to make a complete check-up as those changes may be connected with health problems and mediction in use.
Preparation
As the surgery is done in general anesthesia, all tests recommended by the surgeon and the anesthesiologist are required – mostly laboratory tests with internist examination and ECG.
Ultrasound and mamography are obligatory, as well as hormone analysis – especially sexual hormons.
Surgery
If the mammary gland is big and compact it can be removed through a small incision on the areola border and in case of excessive local fat tissue, it can be removed in the same procedure by lipoaspiration.
In cases where the mammary gland is smaller but with a big amount of fat tissue, the whole procedure can be done by lipoaspiration – fat and glands are partially removed by vacuum-aspiration.
There is a third situation when the breast is so big that it reminds woman breast, with excessive skin, enlarged areola and lowered nipples. In such cases a surgical method similar to the one applied for breast reduction in women is performed in a male patient, with necessary modifications.
Recovery and results
After the surgery drains are removed the following day, and a special brassiere must be worn for 4 weeks. Stitches are being removed between the 10th and the 14th day, and harder physical activities and carrying of heavy things is prohibited for at least one month.
Results in terms of volume reduction and better position are immediately visible, but the final result will appear in a few weeks to few months period when the swelling disappears completely, and skin and tissue take their final shape.
Scars fade gradually in the next few months.
Possible complications
Swelling and bruises, temporary hyper sensibility or numbness, possible asymmetry as well as wrinkles around the nipples will straighten with time.
As an extremely rare complication is the possibility of necrosis.
